Instructions to Complete Accessibility Forms:
If you have been asked to provide a form to the Student Services Department, please select the appropriate form, complete by typing in the required information and provide the “Digital Signature” following the prompts provided. You may e-mail it after completion to the Student Services Department*.
*Please note that the default e-mail is to the Student Services department at the Main Windsor Campus. If you are attending the Chatham Campus or St. Clair Centre for the Arts-Downtown Campus, please enter the correct address (see below) to ensure that the information gets to the correct campus in a timely manner.
