Walk-ins are always welcome, however, to avoid wait times, appointments are recommended. Appointments are required to complete the pre-placement health clearance forms (see below).

Pre-Placement Health Clearance
The Health Centre can be your one-stop shop all of your pre-placement health clearance needs. We can provide immunizations, TB Skin Testing, blood screening for immunity and physicals.
Please download the appropriate forms below and call the Health Centre for an appointment. You may choose to attend the Health Centre at the most convenient location for you.
For students in Community Studies, Health Sciences or other programs where the placement site requests pre-placement health clearance (your coordinator will inform you of this requirement), please download the Medical Requirement Forms. You may have the Health Centre complete the work and forms with you (appointments required) or have your health care practitioner complete the work and forms for you. All forms must be brought into the Health Centre for their review and to obtain a Passport to Health (reviews only - appointments are not necessary)
For students in Nursing or Personal Support Worker, you will be using one of the following forms: Entering Students Paramed Form or Returning Students Paramed Form. For students in the Paramedic Program, you will need to download the following Paramedic Health Forms: Medical Requirements for Entering Students & Paramedic Immunization Form for Entering & Returning Students. You may have the Health Centre complete the work and forms with you (appointments required) or have your health care practitioner complete the work and forms for you. Your forms and blood work will be brought to Paramed according to the instructions provided by your program.
Please note that not all services are covered by OHIP or health insurance and that fees may apply. In an effort to serve all students better, we do our best to keep our fees for service low.
